Unit Clerk - Health Center

Department: Nursing
Location: Rockford, IL

Pay and Schedule Information:

  • Position Pay Scale: $18.50 - $20.25 an hour (depending on experience)
  • Hours: 7:00AM -3:30PM
  • Status: Full-time

General Overview: Coordinates office and clerk activities at the nurses’ station in the Health Center

Essential Functions:

  • Implements direction given by charge nurse in admitting, dismissal and transfer of residents.
  • Answers the phone and makes copies of documentation as needed.
  • Assists as back-up scheduler when necessary.
  • Notifies Social Service Director of resident needs.
  • Sorts and distributes mail.
  • Delivers items to the office.
  • Prints, sorts and mails profile sheets to physician as directed by charge nurse
  • Files information on resident charts as well as thin charts per established policy. Scans information into Matrix EHR.
  • Obtains supplies from nursing storage room, orders doctor envelopes from main office, makes out charge tickets for accounting, stocks resident rooms as directed.
  • Alerts maintenance department of health center needs.
  • Serves as a “runner” for beauty shop, barber, dentist, podiatrist, and optometrist.
  • Checks code alert devices weekly.
  • Charts monthly weights and vital signs.
  • Revises listings such as CNA care sheets and resident census.
  • Creates CNA day shift assignments.
  • Notifies supervisor when supplies are needed, equipment needs repair, and when facilities need further maintenance.
  • Participates in creating an atmosphere, which allows for the privacy, dignity, cleanliness, safety and well-being of each resident.
  • Safeguards privacy and confidentiality of all resident health care information, which includes complying with all HIPAA regulations.
  • Acts as a customer service representative and public relations ambassador for Fairhaven with residents, families, vendors, volunteers and other visitors.
  • Interacts positively with all residents at all times creating an atmosphere, which promotes a homelike environment.
  • Maintains, cares for and safely uses supplies and equipment effectively and efficiently per Fairhaven policies and procedures.
  • Maintains sanitary conditions; follows infection control and waste management procedures as they relate to this position.
  • Reports incidents of abuse, neglect or a violation of the resident’s rights immediately.
  • Protects personal belongings of each resident, reports missing items and assists in attempting to locate them.
  • Completes all in-service training requirements in a timely manner
  • Completes all duties and tasks as assigned.

Essential Qualifications & Requirements:
Type of work and characteristics of duties require:

  • Physical and mental condition allowing for normal office activities including but not limited to hearing, talking, reading, using the computer, writing, mobility to move around the health care center and main building.
  • Ability to use the computer for work assignments.
  • Knowledge of geriatric population in life community setting.
  • Ability to communicate in a friendly manner to residents and coworkers in English.
  • Accuracy.
  • Adherence to attendance policy, understanding residents’ need for 24/7 care


Experience & Education or skills required:

  • High School diploma or equivalent.
  • Good penmanship.
  • Computer software knowledge.
  • Knowledge of medical terminology.
  • Ability to read and follow regulations and instructions
  • CNA certificate (preferred)
